Introductions were made and the June minutes were accepted as written.  Motion to accept the minutes was made by Sherry Combs, second by Carol Bruce-Gage, motion carried.

Executive Committee:

1.  For the upcoming fiscal year, Strong Families/Safe Children has allocated an amount of $109,800 for Gladwin County.  Two weeks ago the coordinator sent an email out to the members of the HSCB, notifying every one of the monies available.  Three of the four contracts had expired this year, which allowed approximately $94,000 available for bids. 

The Executive committee received 7 proposals, which totaled $162,500.  It was a difficult decision, but after looking at the SF/SC contract, it was clear which services met the criteria.  The Executive committee would like those that did not receive funds at this time to wait to talk to the Executive committee during the next scheduled Executive committee meeting.  At that time, questions will be answered, but not until then.  

The Executive committee met yesterday and would like to recommend that the SF/SC money be spent as follows:

$14,898 for the coordinator-this contract is not up until next year.

$1,902 for the administrative funds (meeting expenses and community rep per diem)

$25,000 for Healthy Start services

$32,000 for the COPE services

$28,000 for the FIT services

$8,000 uncommitted

The uncommitted funds will be available for services that are in need of funds throughout the year.  At the next HSCB meeting in August, the coordinator will educate the group on the criteria for receiving funds through Strong Families/Safe Children.  Proposals for the uncommitted funds can then be submitted to the coordinator and the Executive committee will meet to determine if the proposal will be recommended to the HSCB.

At this time, every agency/organization will get one vote.  Each member will state “For” or “Against”, in which the coordinator will record each vote.

Motion to accept the recommendations was made by John Shaffer, second by Tami Jenkinson, motion carried.

2.  The conference, “Bridges out of Poverty:  Strategies for Professionals and Communities”, is being held on August 4, 2005.  A motion to send 7 members of the HSCB was made by Paul Cronstrom, second by Baubette Denison, motion carried. 

Strong Family/Safe Children Coordinator:  An appreciation breakfast for the members of the HSCB will be held on August 16th at the Riverwalk Place in Gladwin.  This will take place of the regular scheduled meeting.

All financial documentation needs to be signed by two designated members of the HSCB.  This will be put on record at the Accounting Department at DHS.  It was suggested that Howard Sweeney and Tami Jenkinson be the 2 designated members, with Sherry Combs being added as an alternative.  A motion was made by Joan Cashin, second by Carol Bruce Gage, motion carried. 

Needs Assessment:  Did not meet.  No presentations were done this past month.

0-6 Interagency:  Health Fair for Gladwin County is being planned for August 16, 2005 from 9-4 pm at the Health Department.  Services to be provided are immunizations, physicals, lead screen, hearing, vision, dental, finger printing and photo ID.  Information from agencies can be put in the information bags.  Items can be dropped off at the NEMCSA Head Start building by August 1st.   

Transition Council:  Did not meet.

Continuum of Care:  A total of $3,198.76 has been spent in Gladwin County.  The breakdown of services follows:  Water $152.28, Electric $1,616.48, Fuel $0.00, and Rent $1,430.00, Foreclosure $0.00.  Gladwin was awarded $31,000 this year for direct services. 

Membership Committee:  80% attendance is encouraged.

 Executive Committee:  A letter of support was given to the Beaverton High School Student Assistance Program for the Grant to Reduce Alcohol Abuse.

A letter of support has also been asked for by Mid Michigan Community Action Agency.  The Executive committee will plan to meet.

 Strong Family/Safe Children Coordinator:  “Workgroup Charges” were updated and handed out.  Updated demographic sheets were updated and handed out.  Updates are being made to the membership list. 

 Clarification—The Gladwin County HSCB will be referred to as the “community collaborative” in state communications and on funding applications.  We have not changed our county name, but can do so if the group sees fit.

 Needs Assessment:  Did not meet.  No presentations were done this past month.

 0-6 Interagency:  Health Fair for Gladwin County is being planned for August 16, 2005.  The 0-6 Interagency group with work with the Kiwanis Club of Gladwin to have one event to take place of the Family Fair.  The health fair is geared for those age 0-6 years old.  The Gladwin County Health Advisory will combine with the Clare County Health Advisory and they will meet on a quarterly basis.   The “Preschool/Kindergarten Here We Come” was held at the Gladwin Elementary School on Tuesday, April 12, 2005 from 6-8pm.  Great Parents/Great Start has videos available at the library.  Books will be donated to the Community Baby Shower. 

 Transition Council:  The Job Olympics were held in February.  The transition grant was sent in and Sandy will know more tomorrow.

 Continuum of Care:  A total of $28,150.22 has been spent in Gladwin County.  The breakdown of services follows:  Water $0.00, Electric $5,922.18, Fuel $6,338.95, and Rent $13,380.09, Foreclosure $2,509.00.

 Membership Committee:  80% attendance is encouraged.

 Meeting Minutes—November 16, 2004

 Present

 Tami Jenkinson called meeting to order at 8:04am.

 Introductions were made.

 Addition:  None

 2005 HSCB Nominations:  Nomination forms were passed out to those that were in attendance.  The nominations will be tabulated and the results will be announced at the December meeting. 

 Executive Committee

 Coordinator’s Report

·         Kara will email all members of the HSCB to find out if any are interested in an appreciation dinner. 

·         The FY 2003-2004 SFSC Plan is complete and will be mailed out this week. 

Workgroup Reports

·         Needs Assessment:  The needs assessment committee will meet after the HSCB meeting on this date.  The needs assessment committee members asked for some direction on where the HSCB would like this assessment to go.  

 A motion was made to have the HSCB adopt the goals that the committee has brought before us.  Motion was made by Karen Blatt, second by Bob Adams, motion carried.

 A motion was made for the workgroup to present the needs assessment to the public and county commissioners and give suggestions on how it can be used to better Gladwin County.  Motion was made by Rose May Mason, second by Joan Cashin, motion carried. 

 ·         0-6 Collaborative:  This workgroup will meet in January and will attempt to refocus and redefine a purpose.   

·         Transition Council:  This years project is going to be an 8th grade job Olympics made up of the special needs classes from the five districts.        

·         Continuum of Care:  To date, $16,815.70 has been spent.  $3,794.04 on electric, $2,601.57 on fuel, $8,911.09 on rent, $1,509.00 on foreclosures.  The continuum of care workgroup would also like to make everyone aware that they are accepting letter of intent to be the fiduciary for the next fiscal year.  The deadline for the letters is December 20, 2004.

 Sharing

·         Coral Beth Rowley:  MSU Extension is offering nutrition classes in the homes of those that are low-income parents.  Project Fresh is finishing up.  The office still has a part time secretary, due to the county budget. 

·         Karen Muma:  The Career Tech Center is looking into a regional Agriculture program for all of the districts, as well as an Entrepreneur program. 

·         Jennifer Miller:  The Shelterhouse will have a woman support group at the office starting in December from 1-3pm.  More information will be sent to local agencies and organizations.    

·         Sherry Combs:  MMCAA is gearing up for Walk for Warmth.  The event will be held in Beaverton this year and will be housed at the First Presbyterian Church Fellowship Hall on February 19th.  The Walk for Warmth is looking for individuals to be members of the steering committee.  The following fundraisers are being planned:  Luncheon/dinners, Burger King on the 1st Friday night, community Tupperware Party, Carter’s community cash, and dress down days at local offices.      

·         Karen Blatt:  The Beaverton Lions are accepting eyeglasses and also any names of those who are in need of eyeglasses.  Please contact Karen at 435.3486.  The Beaverton Ministerial Association will be holding the annual Thanksgiving service on Wednesday, November 24, 2004 at the Beaverton First Presbyterian Church starting at 7pm.  The annual cookie walk will be held on December 10, 2004 at the Beaverton First Presbyterian Church.  The Beaverton Lions will be holding their annual Wild Game Dinner on December 6, 2004 at the Beaverton Community Center; cost is $15 per person. 

·         Paul Cronstrom:  The Gladwin CMH now has a nurse practitioner that will work on Tuesdays and Thursdays. 

·         Joan Cashin:  Beaverton Rural Schools has a declining enrollment and is also working with a budget decrease.  Open enrollment will start at the beginning of the next semester.

·         Maureen Barney-McGuire:  Five new tutors were trained and additional tutors are still needed for Clare County. 

·         Bruce Guy:  A presentation will be done on the Library Needs Assessment at the next scheduled board meeting.  The county library is looking at “Where are we compared to other county libraries?” and also looks at plans for the future.  The Gladwin county library is limited by physical space.  The library should have 18,000 square feet, but only has 8,000.  This has not changed in over 30 years. 

·         Natasha Allen:  Gladwin County Industries is looking at a way to recognize those employers that employ individuals that are developmentally disabled. 

·         Rose May Mason:  The Gladwin City County Transit is looking at a 4% decrease in the upcoming year.  It is estimated that six transits in the state will fail.  Currently the transit is reaching every area of the county 5 times a day. 

·         Carolyn VanWert:  Many patients are dependent on dial a ride.  A new Visiting Nurse and Hospice office will be opening up in the office building next to Pamida.  A new surgeon will be coming soon to take the place of Dr. Rowbar.  The “Interagency Committee” is meeting in January and is looking at the need to continue the committee as it stands. 

·         Anna Freund:  MI Works! is looking for non profit agencies and organizations that will be willing to have youth, age 19-21, work for them at least 30 hours a week. 

·         Robert Adams:  The Gladwin FIA office is looking at losing one to three more staff in the upcoming year.  Michigan also has the 2nd highest error rate when it comes to food stamps. 

·         Tami Jenkinson:  The United Way campaign is underway with a goal of $135,000.  Currently the campaign is slow and is only at 20% of the goal.  The fundraiser at BC Pizza raised $350 for the United Way.  The United Way also has Toys’R’Us coupons for those that would like to shop after hours at the Saginaw Store November 28, 2004.  Tickets are $5 a piece and all of the proceeds go to the United Way of Gladwin County.

Meeting Minutes—September 21, 2004

Introductions were made.

Acceptance of August minutes: Motion made by Bev Przystas, second by Sherry Combs, motion carried.

Addition:  Deb Snyder-Great Parents/Great Start

            The Great Parents/Great Start grant is due October 1, 2004.  Deb is looking for the support of the HSCB.  The grant amount is for $20,000 that will be used to dispense ‘Wonder Years’ and community happenings, provide services to teen parents, and also allow more home visits.  John Haycook made a motion to support the grant after Deb goes through the proper procedure of the HSCB, Bev Przystas second, motion carried. 

Poverty Training

Seven members from the HSCB will he attending this training/workshop on October 19, 2004.  The HSCB will pay the registration fee for those members who volunteered to go.  The members are as follows:  John Haycook, Amy Pratt, Tami Jenkinson, Baubette Denison, Bev Przystas, Arlene Krager, and FIA staff member

Coordinator’s Report

·         Kara suggested that the group spend some of the administrative money on a dinner for the members of the Gladwin County HSCB.  There was no discussion.

·         SF/SC Contracts for the FY 2005 are ready for signatures.

Workgroup Reports

·         Needs Assessment:  The needs assessment workgroup have set some goals for the HSCB to work on. (see attachments in email) 

Short Term Goals

1.      Maintain a quality sufficient safety net for the next twenty years.

2.      Maintain a quality sufficient safety net thereafter, which will experience a much smaller volume.

Long Term Goals

1.      Exponentially increase funding for public education activity 0-18 years.

Sharing

·         Amy Pratt:  NEMCSA Head Start and Midland Community Foundation are having a family fun day for the children and their families on October 9, 2004 at the Maxwell Pumpkin Farm. 

·         Karen Muma:  The Career Tech enrollment is up.  Any information that you would want to give to the K-8 Counselors can be given to Karen. 

·         John Shaffer:  The EMS did some car seat safety checks at the Family Fair.  There will also be a renewal for EMS services on the ballots.  

·         Sherry Combs:  MMCAA is gearing up for Walk for Warmth.  The event will be held in Beaverton this year and will be housed at the First Presbyterian Church Fellowship Hall.  Rod Wagner is no longer with the Red Cross in Midland and Gladwin Counties.     

·         Karen Blatt:  A Roast Beef Dinner will be held on October 7, 2004 form 4 pm-7pm at the First Presbyterian Church.  “Meet the Candidates” will be held on October 12, 2004 form 7-0 pm at the Gladwin VFW.  The Beaverton Lions are excepting eyeglasses and also any names of those who are in need of eyeglasses.  Please contact Karen at 435.3486.

·         Paul Cronstrom:  The CMH clinic in Harrison will be holding a leadership committee meeting on October 27, 2004.  The meeting will provide information on co-occurring individuals.  Big Brothers/Big Sisters is looking for board members in Clare County.  There will be a Spaghetti Dinner at the Harrison Moose on November 14, 2004 form 2-6pm. 

·         Nancy Doyle:  October 1, 2004 brings changes for the LEAP direct program.  If a customer has a dependent child and receives food assistance, they maybe eligible to have their consumer energy shut off notice covered.  Consumer Energy will set up a payment plan with customers and the customers will benefit by paying the monthly payments and receive a % off. 

·         Bev Przystas:  A two-part satellite service is being offered.  A web based nutrition and fitness class is being offered through MSU Extension called “Jump Through JIFF”.  For more information go to the following web site.    http://www.msue.msu.edu/cyf/youth/jiff/JIFFcourse/index.html The library had a needs assessment done and it will be presented at the Gladwin Library on October 19th

·         John Haycook:  NEMSAS is going in a new direction for the assessments, which should result in a smooth manner.  John is encouraging two bills to pass which deal with substance abuse and mental illness.  John will get the bills numbers to Kara for sharing. 

·         Marti Miller:  November 2, 2004 is Election Day.  On the ballots will be a vote for a 19% surcharges for E911, which will increase your phone bill by $3.20, a line. 

·         Robert Adams:  Bob strongly encouraged that the members of the HSCB take back the information presented at the HSCB meetings to your local agencies.  The Child Death Review Team met and found that most deaths were due from risky behaviors and poverty issues. 

·         Tami Jenkinson:  The United Way campaign is underway with a goal of $135,000.  The United Way also has Toys’R’Us coupons for those that would like to shop after hours at the Saginaw Store November 28, 2004.  Tickets are $5 a piece and all of the proceeds go to the United Way of Gladwin County.

            Due to funding issues the Department of Community Health will cap enrollment in the Adult Medical Program effective July1, 2004.  This program pays for basic medical care such as doctor visits, medical testing and prescription coverage for low-income adults.  This would include individuals that do not meet the definition of disability according to our Medical Review Team for Medicaid.  Some of these individuals are certified as unable to work due to a mental or physical disability for at least 90 days or are receiving services from Michigan Rehabilitation Services.  They also might be in a Substance Abuse and Treatment Center, have diabetes, high blood pressure, mental heath issues, joint disease, or other various medical conditions. 

            Therefore our most serious concern is the availability of ongoing prescription coverage; however there will also be emergency situations in which a customer will need assistance.  FIA is bringing this to the table to determine all available community resources and compile a list to be available should the need arise.

2.      Maintain a quality sufficient safety net thereafter, which will experience a much smaller volume.

Long Term Goals

1.      Exponentially increase funding for public education activity 0-18 years.

·         John Shaffer:  A Child Safety Seat Check was held on Tuesday, July 13, 2004 from 4-7pm at the Gladwin Fire Station.  19 seats were checked and 9 were replaced. 

·         Sherry Combs:  MMCAA is training to do income taxes.  There are slots that could be filled for commodities and weatherization.  MMCAA will be represented at the fair by using the United Way booth.

·         Maureen Barney-McGuire:  Numbers are climbing and referrals are up.  Maureen offered to speak to local agencies regarding the Literacy council.  Maureen has email address change cglc@ejourney.com

The literacy council is holding a raffle for several items $1per ticket.  The literacy council will also be represented at the fair by using the United Way booth.

·         Bruce Guy:  The library needs assessment will begin and an outside consultant has been hired.

·         Marti Miller:  Re-elections are coming soon.  The commissioners asked each department for a 7% decrease.

·         Carolyn Van Wert:  Carolyn will be taking over the discharge planning and will have a RN to assist her with her job duties.  A new physician is in Gladwin who specializes in internal medicine.  The Gladwin Hospital is offering MRI services, cardiac rehab, and massage therapy. 

·         Joan Cashin:  A summer school program was offered this summer.  100 students grades k-8 attended daily.  A new boiler is being installed at the primary school.

·         Tami Jenkinson:  The Annual Golf Outing will be held on Friday, August 13, 2004 at Gladwin Heights Golf Course.  The fee is $240 per team.  There will be a United Way booth at the fair for any non-profit agency to use.  Hospice also has a quilt that will be raffled, tickets are $1.

·         Paul Cronstrom:  CMH is working on referring those on psychotropic medications to their family doctor, unless their mental illness is severe.  CMH will first recommend therapy and then psychotropic medications.

·         Tamie Robinson:  Kindergarten round up is over and approximately 120 students will be attending kindergarten next year in Gladwin.  There may be building expansions with the renewal of a bond for Gladwin Schools as well.    

·         Sherry Combs:  Walk for Warmth fundraisers have brought in $248, which has brought the total up to the $10,000 mark. 

·         Amy Pratt:  Recruiting and enrolling are still going on.

·         Karen Muma:  High School enrollments are taking place for the Tech Center.  The Counselors held a meeting in which CMH attended. 

·         Karen Blatt:  Six Deacons are looking for local information and would like to meet with some agency, organization and community leaders.  Karen would appreciate any volunteers. 

·         Maureen Barney-McGuire:  Maureen has no computer at this time.

·         Bruce Guy:  Friends of the Library are holding a quilt raffle.

·         Joan Cashin:  There is a reduction in programs and services for the Beaverton Schools next year, with a few more cuts that need to be made.  June 14th the polls will be open for a school vote worth 1.7 million.  Three people are running for 2 school board positions. 

·         John Haycook:  Human Aid is always available for presentation and to meet with individuals. 

·         Bev Przystas:  Project Fresh will be starting in June.  The Gladwin County Fair is July 19-24.  A group from 4-H is heading to Washington D.C.  There is a lot of soil testing being done at this time.  The Master Gardener will start in August. 

·         Robert Adams:  Staffing allocations came out in April with Midland losing 5 employees and Gladwin losing 2. 

·         Paul Cronstrom:  CMH has two Full Time Peer Case Managers working in Gladwin and Clare.

·         Tami Jenkinson:  The Annual Golf Outing will be held on Friday, August 13, 2004 at Gladwin Heights Golf Course.  The fee is still undetermined.

·         Anna Freund:  A job fair will be held at the Doherty in Clare on Thursday from 9-1pm.

·         Paul Cronstrom:  A formal access system is being developed in Gladwin at CMH.

·         Tami Jenkinson:  Welcome Carolyn VanWert, who will be replacing Lenore Klassen.  United Way has awarded Kathy Wilton with the Life Time Achievement award.  The United Way golf outing is tentatively set for August 13, 2004.

·         Tami Jenkinson:  March 11, 2004 was the Annual United Way meeting.  The first halves of the allocations were awarded.  The youth volunteer of the year was Kelly Fitzpatrick.

·         Sherry Combs:  Walk for Warmth will be having a spaghetti and meatball dinner on Saturday, March 27th.  The mayor of Gladwin has claimed February 21st as the Walk for Warmth day.  Nearly $8000 has been raised for Walk for Warmth.

·         Mark Boyce:  Still looking for a building to host the “Personal Needs Pantry”.

·         Marti Miller:  Emergency Management has been busy with the rising of the lakes and rivers in the area.  The Sheriff’s Department received 100% on their inspection.  Commissioners are campaigning with primary elections being August 3, 2004 and the general election being held November 2, 2004.

·         Deb Snyder:  The RESD will be sponsoring a Great Parents/Great Start workshop this summer.

·         Amy Pratt:  NEMCSA is enrolling for next year. Call Amy or Janelle Cuddie with referrals. 

·         Karen Blatt:  A community Good Friday service will be held in Beaverton at the Church of the Nazarene.  Legislative breakfasts will be held on March 19th in Sugar Springs and on April 2nd at the First Presbyterian Church in Beaverton.  FORGE is still going forward with the recycling process.

·         Maureen Barney-McGuire:  A mother-son dance was held in Harrison.  80 were in attendance.  Student referrals are up. 

·         Bruce Guy:  There will be a presentation on Need Assessments for Libraries.  The presentation will be held on March 30, 2004 at 7pm in Room 10 of the sheriff’s department.

·         Karen Muma:  Career Day for the 5 schools in the district was held on March 11, 2004 at the Tech Center.  There were approximately 800 students that attended. 

·         Paul Cronstrom:  While the state did not reduce CMH funding, CMHCM is needing to look at how to subsidize funding for home help.  This will impact any additional service expansion for CMHCM for this year.

·         Natasha Allen:  GCI has a crew doing the inserts at the Gladwin County Record, a crew cleaning the Record and also helping with the M & K deliveries. 

·         Joan Cashin:  Thank all who helped with the trying times with the road in the school district.

·         Sherry Combs:  Gearing up for “Walk for Warmth” on February 21st.

·         Robert Adams:  The COPE program through CMH and the courts has been a great success in Gladwin county.  This program actually saves the county money.  It is unknown at this time if there will be any staff cuts in the Gladwin and Midland offices. 

·         Marti Miller:  The county controller is gone.  The commissioners will be having meeting with the department heads. 

·         Bev Przystas:  4-H is having a tubing party.  Annual Farmer Appreciation day is also coming up. 

·         Amy Pratt:  NEMCSA is enrolling for next year.  There are approximately 20 openings.  Call Amy or Janelle Cuddie with referrals. 

·         Karen Blatt:  The next legislative breakfast will be held in Sugar Springs on March 19, 2004 at 7 am.  FORGE has a board meeting on March 1 at the MSU Extension office at 7 pm.

·         Bruce Guy:  Planning summer reading program and looking for alternative funding.

·         Karen Muma:  Career Day for the 5 schools in the district will be held on March 11, 2004 at the Tech Center.  The students are finishing up houses in Harrison and Clare.  

·         Paul Cronstrom:  CMH is utilizing best practice and is voting on adopting a COPE program in Clare County.  The COPE program will keep out of home placement down. 

·         Tim Verity:  The ministerial association had the sign stolen from the nativity seen located on the courthouse yard.  The Gladwin Ministerial association is also sponsoring a habitat for humanity home.

·         Darrell Schlese:  It is recommended that Gladwin County have another Circuit Judge.

·         Karen Blatt:  The second legislative breakfast will be held at the Beaverton Presbyterian church on February 13, 2004 at 7 am.

·         Joan Cashin:  Joan accepted the position of Superintendent for the Beaverton Schools.  The primary school is going through some boiler renovations. 

·         Karen Muma:  Career Day for the 5 schools in the district will be held on March 11, 2004 at the Tech Center.

·         Paul Cronstrom:  There is a need for children psychiatric services and CMH is looking at having a nurse practitioner fill this position.
